Our Pledge Regarding Your Health Information
We are required by law to maintain the privacy of your protected health information (PHI), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of this Notice currently in effect.
How We May Use and Disclose Your Health Information
Treatment. We may use your PHI to provide and coordinate your care, including sharing it with other providers involved in your treatment.
Payment. We may use and disclose your PHI to obtain payment for the services we provide.
Healthcare Operations. We may use your PHI for our internal healthcare operations, such as quality assessment and staff training.
As Required by Law. We may disclose your PHI when required to do so by federal, state, or local law.
Your Rights
You have the right to:
- Inspect and copy your health record
- Request amendments to your record
- Receive an accounting of disclosures
- Request restrictions on certain uses and disclosures
- Request confidential communications
- Receive a paper copy of this Notice
- File a complaint with us or with the U.S. Department of Health and Human Services

This Notice is a standard NPP framework. Final language including the effective date, Privacy Officer contact, and any state-specific provisions must be reviewed and approved by the practice and counsel before go-live.
