Most new patients complete their forms by phone or at the start of their first visit. If you would like paperwork sent ahead of time, we will email or mail it to you.

What we collect

Less paperwork than you expect.

We keep new-patient paperwork brief. Our team gathers most of what we need during the first phone call, and the rest is completed at your evaluation visit. If you prefer to fill things out at home, we are glad to send them to you.

  • Intake and history

    Your demographic information, current medications, primary care provider, and a brief history of what brings you in. We collect this so your clinician can prepare for the visit and keep the time focused on you.

  • Insurance and consent

    A copy of your insurance card and ID, plus standard consent to treat. We verify your benefits before the first visit so you know your coverage in advance. See Insurance We Accept for the full list of plans.

  • HIPAA and privacy

    Acknowledgment of our HIPAA Notice of Privacy Practices and any release of information you would like in place. See our HIPAA Notice for the full text.

How it gets to you

Two ways to get your paperwork.

By phone

Call us at 301-710-4218 Monday through Saturday and we will email or mail the right forms to you the same day.

By message

Use our contact form and note in your message that you would like new-patient paperwork in advance.
