What Depression is , and what it is not.
Depression, also called major depressive disorder, is a common and serious medical illness that affects how you feel, think, and handle daily life. It is more than a passing low mood or a hard week. To meet the clinical definition, symptoms last most of the day, nearly every day, for at least two weeks, and represent a change from how you usually function.
Depression is not a sign of weakness, and it is not something you can simply decide to stop. It involves real changes in the brain and is shaped by a mix of biological, genetic, environmental, and psychological factors. It affects people of every age and background, including children and older adults, and it often appears alongside anxiety, chronic pain, or a substance use problem.
The most important thing to know is that depression responds well to treatment. With an accurate diagnosis and a plan matched to you, most people improve. Reaching out is the first clinical step, not a last resort.
What it can look like.
Depression looks different from person to person, and only a comprehensive evaluation can diagnose it, but these are the signs clinicians look for.
- A persistent sad, empty, or hopeless mood most of the day, nearly every day
- Loss of interest or pleasure in activities you used to enjoy
- Changes in appetite or weight, up or down, not tied to dieting
- Trouble sleeping, or sleeping much more than usual
- Fatigue or a loss of energy that rest does not fix
- Trouble concentrating, remembering, or making decisions
- Feelings of worthlessness or excessive, inappropriate guilt
- Restlessness, irritability, or a noticeable slowing of movement and speech
- Unexplained aches, pains, headaches, or digestive problems
- Thoughts of death or suicide, or that you would be better off not here
How we treat Depression at Oasis of Hope.
Care begins with a comprehensive psychiatric evaluation. Depression cannot be diagnosed from a checklist or a single rushed visit, so your first appointment is a 60 to 90 minute conversation about your history, what you are experiencing now, and what you want to feel different. Where they add clarity, we use validated screening tools such as the PHQ-9. You leave with an initial diagnosis where one is warranted and a plan you understand and agree with.
For many people, treatment combines medication management and psychotherapy, and the evidence supports that combination for moderate to severe depression. Our medication management is led by Charlotte Ayuk-Nkem, APRN, CRNP-PMH, a Psychiatric Mental Health Nurse Practitioner, who finds the right antidepressant at the right dose and adjusts it with you over follow-up visits. Medication is a tool, not the goal, and the decision to use it is always yours, made with accurate information.
Therapy does the work medication cannot. Through psychotherapy, we match the approach to you, often cognitive-behavioral therapy, which targets the negative thought patterns depression feeds on, or interpersonal and supportive therapy for the relationships and self-esteem it erodes. Some people do well with therapy alone; for others, medication and therapy together work best. We decide the mix with you, not for you.
